Why after age 45 this is often a better choice than laser

Laser correction reshapes the cornea but does not touch the natural lens – and it is the lens that hardens with age and is responsible for presbyopia. After fifty, laser therefore solves only part of the problem, and cataract develops anyway in a dozen or so years.

One procedure instead of two stages: we replace the natural lens with a premium implant – we correct refractive error and presbyopia simultaneously.
Cataract ceases to be an issue: the implanted lens does not cloud. The disease that affects almost everyone after seventy will no longer occur.
Stable result: the lens does not change with age the way the cornea does after laser correction in patients with progressive presbyopia.

Honestly: this is an intraocular procedure, so we weigh indications more carefully than with laser. In persons under 45 with preserved accommodation we usually advise against RLE and recommend laser correction or phakic ICL lenses.

Do I qualify for lens exchange?

Green light: age usually above 45–50 years, refractive error with accompanying presbyopia, early changes in the natural lens, healthy retina.
Yellow light – requires assessment: glaucoma, diabetes, high myopia (risk of retinal detachment requires separate discussion), AMD changes.
Red light: young age with preserved accommodation, active eye inflammation, unrealistic expectations about vision without glasses in every situation.

How the procedure works in Kraków

1. Qualification – in Kraków: approx. 90 minutes (400 PLN); biometry, MS-39 tomography, fundus assessment, lens power calculation and model selection.
2. Surgery – approx. 15 minutes per eye: topical anesthesia, outpatient. We remove the natural lens by phacoemulsification and replace it with a premium implant.
3. Second eye: at an interval – which is why we charge per eye.
4. Follow-ups – in Kraków: first one the next day, subsequent ones according to schedule; 2 follow-ups included in the package.

Trifocal or EDOF – how we select the lens

This is the most important decision of the entire procedure and there is no single best implant. There are two different vision profiles, and the choice depends on how you actually use your vision.

Trifocal lens – full range of focus: distance, intermediate and reading. The price for this: light effects (halos) may appear around lights at night. This is not a defect of the implant but the physics of light refraction on multifocal rings. They usually diminish during neuroadaptation.
EDOF lens – extended depth of focus instead of separate focal points. Very good contrast and minimal light effects at night, making it a sensible choice for drivers and people who work extensively at monitors. With fine print, some patients use weak reading glasses.
Diagnostics and lifestyle determine the choice. A professional driver, photographer and person who reads several hours a day need different implants. MS-39 maps show what the eye’s anatomy permits at all – the conversation about daily life settles the rest.

Healing and neuroadaptation

First days: vision improves quickly; we protect the eye from rubbing.
2 weeks: no intense exertion or sports.
4–6 weeks: complete healing of the eye – neuroadaptation of the brain to the new lens optics proceeds in parallel and takes longer.
Week 2–12 – adjustment: the brain must adjust to the new vision – this takes time and is completely normal.
Month 3–6 – full adaptation: neuroadaptation comes to an end. If it proves necessary, this is precisely when we perform fine-tuning.

Think of hypertension therapy: we start with one tablet, measure blood pressure again – and adjust the dose until the result is right. It is the same with presbyopia correction: we correct, the brain adjusts to the new vision, and if necessary – we fine-tune. Enhancement is not a failure but an element of precise treatment.

Risks – honestly

Refractive lens exchange is an intraocular procedure with a safety profile similar to cataract surgery – one of the best-studied procedures in medicine. Possible consequences include: secondary cataract (removed with YAG laser, 600 PLN), transient corneal edema, increased intraocular pressure, and with high myopia an elevated risk of retinal detachment – we discuss this separately before the decision. With multifocal lenses, light effects at night are possible during the adaptation period.

Frequently Asked Questions

Will I still need glasses after the procedure?

It depends on the type of implant chosen. Trifocal lenses usually provide independence from glasses at all distances, including reading. EDOF lenses give very good distance and intermediate vision; with fine print, some patients reach for weak reading glasses. Which variant is right is determined by diagnostics and your lifestyle.

Can I get cataracts after the procedure?

No. The implanted lens does not cloud like the natural lens. However, the lens capsule may cloud (so-called secondary cataract) – it is removed with a few-minute YAG laser procedure.

How does RLE differ from cataract surgery?

Technically it is the same procedure. The difference is in the indication: with cataract we operate on a clouded lens, with RLE – on a clear lens, in order to correct refractive error and presbyopia.

From what age is it worth considering RLE?

Usually above 45–50 years, when presbyopia accompanies refractive error. In younger patients with preserved accommodation we rather recommend laser correction or phakic ICL lenses.

Is refractive lens exchange reimbursed by the NFZ?

No. The NFZ finances cataract surgery with a monofocal lens when cataract is already diagnosed. Refractive lens exchange is a private procedure: we perform it before cataract develops, and we implant a premium lens that also corrects presbyopia. In Kraków it costs 11,000 PLN per eye; installments are possible with Santander Consumer Bank.

How long does the procedure take and does it hurt?

The procedure itself takes about 15–20 minutes per eye and is painless – we anesthetize the eye with drops, you are conscious and feel only touch. We operate on the second eye after a few days to a few weeks, in accordance with the safety standard protecting against infection.

Is it worth having lens exchange instead of laser correction after age 45?

Are both eyes operated on the same day?

No. We perform lens exchange with an interval of a few days to a few weeks between one eye and the other. For patients from outside Kraków we plan both appointments so that the number of visits is minimized.

For how many days will I receive medical leave?

Usually 3 to 7 days per eye – that is how long is needed for safe healing before returning to work, especially at a computer.

Will I see halos around lights at night after the procedure?

With multifocal lenses, halos at night result from their ring construction and in most patients diminish within weeks to months of neuroadaptation. For people who drive a lot at night, we more often recommend an EDOF lens, which produces fewer light effects at the cost of reading the finest print.

Will the lens also correct astigmatism?

Yes. With astigmatism we implant a toric variant with a built-in cylinder axis – at SwissLaser at no extra charge; a premium implant always costs 11,000 PLN per eye.

Can I choose the type of lens myself?

You can have a preference, but we select the model after eye biometry and a conversation about your lifestyle: a trifocal lens gives a full range of focus, EDOF smooth distance and intermediate vision with less halo, monofocal the clearest distance image with reading glasses. All variants cost the same.

How many times do I need to come to Krakow?

Usually: qualification examination (about 90 minutes), first eye surgery with follow-up the next day, second eye surgery with follow-up, then follow-ups in the first month included in the package. Some follow-ups can be performed by arrangement with an ophthalmologist closer to home. From Katowice it is about an hour via the A4 motorway, from Kraków Główny station 15–20 minutes to the clinic.

What is secondary cataract and is there a risk after lens exchange?

The implant does not cloud, but in some patients after months or years the capsule in which the lens sits clouds slightly. We remove this with a YAG laser in the office – the procedure takes about a minute, is painless and restores full sharpness.

Pierwsza kontola: Na pierwszą kontrolę przyjechałem następnego dnia rano po dokładnie 16-stu godzinach od zabiegu i już wtedy widzenie miałem na 99%. Dziś:27.07.2026 r.właśnie wróciłem z drugiej kontroli moim zdaniem najważniejszej. Efekt-widzenie na prawe oko skorygowane do dali-150% Widzenie na lewe oko skorygowane do bliży 100%. Reasumując: Chciałem serdecznie podziękować całemu zespołowi Kliniki Swisslaser począwszy od pań na recepcji poprzez panią optometrystę panią doktor kwalifikującą mnie do zabiegu,pana doktora z pierwszej kontroli po zabiegu a w szególności panu doktorowi Victorowi Derhartunianowi.
