Why this is often the better choice than laser after 45

Laser correction reshapes the cornea. It fixes the refractive error but leaves the eye’s own lens untouched – and it is precisely that lens which loses elasticity over the years and clouds later. Someone who has laser at 50 corrects one problem and keeps two.

Lens exchange works one level deeper. The ageing lens is gone, and with it presbyopia and the prospect of cataract. So the question is not “laser or lens”, but: what condition is your own lens in today?

Am I suitable for lens exchange?

Green light: usually from 45 to 50 years, refractive error accompanied by presbyopia, early changes in the natural lens, healthy retina.
Amber – requires assessment: glaucoma, diabetes, high myopia (the risk of retinal detachment deserves a separate conversation), macular degeneration changes.
Red light: young age with preserved accommodation, active intraocular inflammation, unrealistic expectations of complete spectacle independence in every situation.

How the procedure works in Kraków

1. Suitability assessment: about 90 minutes (400 zł) – optical biometry, MS-39 tomography, fundus examination, calculation of lens power and choice of model.
2. Surgery – about 15 minutes per eye: under topical anaesthesia, day-case. The natural lens is removed by phacoemulsification and replaced with the premium implant.
3. Second eye: at a later date – which is why we bill per eye.
4. Follow-up: two check-ups in the first month, included in the package.

Before the assessment we ask for a contact lens break: soft lenses at least seven days, rigid lenses at least four weeks.

Trifocal or EDOF – how we choose the lens

This is the most important decision of the whole procedure, and there is no single best implant. There are two different visual profiles, and the choice depends on how you actually use your eyes.

Trifocal lens – the full range of focus: distance, intermediate and reading. The price for that: halos may appear around lights at night. This is not a fault of the implant but the physics of light refracted by multizonal rings. They usually diminish during neuroadaptation.
EDOF lens – extended depth of focus instead of separate focal points. Very good contrast and minimal light phenomena at night, which makes it a sensible choice for drivers and screen work. For fine print some patients use weak reading glasses.
Diagnostics and daily life decide.A professional driver and someone who reads for several hours a day need different implants. The MS-39 maps show what the anatomy permits; the conversation about daily life settles the rest.

Healing and neuroadaptation

First days: vision improves rapidly; the eye is protected from rubbing.
Two weeks: no intense exertion or sport.
4–6 weeks: the eye is fully healed.
Weeks 2 to 12: the brain adjusts to the new optics. This neuroadaptation runs in parallel and takes longer than the physical healing.

Risks – stated plainly

Refractive lens exchange is an intraocular procedure with a safety profile comparable to cataract surgery – one of the best-studied procedures in medicine. Possible sequelae are: posterior capsule opacification, removed in a few-minute YAG laser procedure (600 zł per eye), transient corneal oedema, a rise in intraocular pressure and, in high myopia, an increased risk of retinal detachment.

And an honest note: no lens guarantees complete spectacle independence in every situation. With very fine print or in low light some patients still reach for weak reading glasses – that is a predictable part of the outcome, not a complication.

When is lens exchange better than laser correction?

Can a cataract still develop after the procedure?

No. An artificial lens does not cloud, so a cataract can no longer occur. Any later opacification of the capsule – posterior capsule opacification – is treated in a few-minute YAG laser procedure (600 zł per eye).

Will I still need glasses after the procedure?
