Three pathways – which to choose
- PresbyMAX (laser): the AMARIS laser creates a gentle multifocal profile on the cornea. For patients with a clear natural lens, typically between ages 45 and 55. The procedure takes approximately 15 minutes for both eyes. Cost: 15,000 PLN.
- Refractive lens exchange: we replace your natural lens with a premium implant. Advisable when the lens begins to harden or cloud – it resolves presbyopia and eliminates the possibility of future cataract. Cost: 11,000 PLN per eye.
- Monovision: one eye set for distance, the other for near. A solution for individuals who tolerate this asymmetry well – we test this beforehand with contact lenses.
The principle we follow: as long as your natural lens is clear and healthy, there is no reason to replace it – in that case we recommend laser. When the lens begins to change, replacement is a lifelong solution and simultaneously closes the chapter on cataract.
Am I a candidate?
- Green light: stable refractive error, healthy cornea and retina, realistic expectations – the goal is independence from glasses in everyday situations, not perfect vision under all conditions.
- Yellow light – requires assessment: dry eye syndrome, thin cornea, glaucoma, work requiring extreme contrast at night (professional drivers, pilots).
- Red light: active eye disease, keratoconus, unrealistic expectation of “vision like at age 20.”
For night drivers and individuals working at screens, we more often recommend EDOF lenses, which provide smooth vision with minimal light effects.
How treatment proceeds in Warsaw
- 1. Qualification examination – Warsaw: approximately 90 minutes (400 PLN); MS-39 tomography, biometry, assessment of the natural lens, and simulation of multifocal vision.
- 2. Procedure: PresbyMAX – approximately 15 minutes for both eyes, outpatient. Lens exchange – approximately 15 minutes per eye; eyes are operated at intervals.

Neuroadaptation – the most important thing you need to know
Presbyopia correction always means that the brain must learn to use the new image. This takes longer than after standard laser correction.
- Week 2–12 – adjustment: the brain must adjust to the new vision – this requires time and is completely normal. Vision improves gradually.
- Month 3–6 – full adaptation: neuroadaptation comes to an end. If it proves necessary, this is when we perform precise fine-tuning.
Think of hypertension therapy: we start with one tablet, measure blood pressure again – and adjust the dose until the result is right. It is the same with presbyopia correction: we correct, the brain adjusts to the new vision, and if necessary – we fine-tune. Enhancement is not a failure, but an element of precise treatment.

Risks – honestly
With any multifocal correction, some patients experience light effects at night or reduced contrast during the transition period – these usually resolve with neuroadaptation. After PresbyMAX, temporary dry eye is possible. With lens exchange, the risks of intraocular surgery apply. The key is thorough qualification and honest management of expectations – which is why we discuss in detail what to expect before the procedure. Enhancement after laser surgery is included in our post-procedure care (warranty).

The main adjustment phase falls between weeks 2 and 12, and full neuroadaptation usually ends between months 3 and 6. This is a normal process, not a complication.
PresbyMAX or lens exchange – how do we choose in Warsaw?
Age, the condition of the natural lens, and expectations are the deciding factors. Between ages 45 and approximately 55, with a clear lens, we usually recommend PresbyMAX. When the first changes appear in the lens or the patient wants to close the chapter on cataract once and for all, lens exchange with a premium implant is the better choice.
Does laser still make sense after age 60?
Rarely. After sixty, the natural lens usually loses clarity, so corneal correction solves only part of the problem, and cataract develops within a few years. At this age, we most often recommend lens exchange – one procedure instead of two.
How long does neuroadaptation take and what do I see during this time?
The brain learns the new vision over several weeks to several months. Halos at night and fluctuations in near clarity are possible, which gradually diminish.
Does the NFZ (National Health Fund) reimburse presbyopia treatment?
No. Neither laser nor lens exchange with a multifocal implant is funded by the NFZ. Both procedures can be paid in installments in Warsaw.
How many visits in Warsaw does the treatment involve?
