Phakic lens price at a glance
- EVO ICL / EVO+ lens procedure: 11,000 PLN per eye. The same price for spherical and toric variants (Myopic, Myopic Toric, Hyperopic, Hyperopic Toric), in Warsaw and Kraków.
- Lenses outside the standard range: for astigmatism above 6 D, very high myopia or hyperopia, we order a customized lens – price upon request.
- Included in the price: a lens individually ordered to your eye’s parameters, procedure performed by Dr. med. Victor Derhartunian, post-procedure drops according to schedule, and two follow-up appointments in the first month.
- Assessment: 400 PLN, approximately 90 minutes, with MS-39 tomography and digital lens size selection. Paid separately, because only after it do we know if the procedure is right for you.
- Pair of eyes: 22,000 PLN, standardly during one session (separately upon request). The procedure is not reimbursed by NFZ; instalments are possible (Santander Consumer Bank), calculator below.

There are no hidden surcharges for “technology,” “premium lens,” or toric variant. If the assessment shows that laser or lens exchange is a better solution, we will tell you directly and charge the procedure according to the appropriate price list.
Phakic lenses – price in Poland compared to the market
In Poland, you will pay from approximately 6,600 to 12,800 PLN per eye for phakic lens implantation. The range results from three things: the lens model (older anterior chamber lenses are cheaper than posterior chamber EVO ICL with a central opening), surcharge for the toric variant (usually 1,000–2,800 PLN), and whether follow-ups and drops are included in the price.
- Lens: EVO ICL / EVO+ is the current posterior chamber generation; it does not require iridotomy and has a UV filter. Cheaper offers often concern other models.
- Astigmatism: with us, the toric variant is included in the price. For astigmatism, compare the price of a toric lens, not a spherical one.
- Diagnostics: we select the lens size digitally using MS-39 tomography. Incorrect size is the most common cause of repeat procedures, so this item genuinely reduces the risk cost.
- Surgeon: approximately 1,500 phakic lens implantations and over 25,000 refractive procedures. The procedure is performed by the same doctor who qualifies you.

Instalments, payment, and formalities
- Instalments: Santander Consumer Bank, decision usually on the same day; you can calculate the instalment amount with the calculator above. Formalities are handled at the clinic.
- Payment: bank transfer, card, or cash; you pay for the procedure before the operation date, assessment on site.
- NFZ: phakic lenses are not reimbursed. A personal invoice for deduction as part of rehabilitation relief is only possible with a disability certificate; in other cases, it is not.
- Patients from outside Warsaw and Kraków: we plan the assessment and procedure to limit the number of visits; the follow-up the next day takes place at the clinic.

Phakic lens price list – questions and answers
How much do phakic ICL lenses cost at SwissLaser?
11,000 PLN per eye in Warsaw and Kraków, one price for spherical and toric variants. Assessment 400 PLN.
Is there an extra charge for a toric lens (astigmatism)?
No. The toric EVO ICL variant costs the same as the spherical one with us. In many clinics, the surcharge is 1,000–2,800 PLN.
Are phakic lenses reimbursed by NFZ?
No. This is a private procedure. Instalment payment through Santander Consumer Bank is possible.
Does the price include post-procedure follow-ups?
Yes, two follow-ups in the first month and post-procedure drops. Annual check-ups with vault and endothelium measurement are charged as a visit.
Why is the assessment paid separately?
Because only after the 90-minute examination do we know whether a phakic lens is right for you, or if laser or lens exchange would be better. You pay for an honest answer, not for selling a procedure.
How much does phakic lens implantation cost for both eyes?
22,000 PLN. We standardly operate on both eyes during one session; separately upon request.
