Why the question “which method is best” has no single answer

Each method solves the same problem – refractive error – but through a different path and with a different compromise. A method that is optimal for one patient may be contraindicated for another. The decision is not based on trends or price, but on the anatomy of the specific eye.

Therefore, the sequence is always the same: first diagnosis, then narrowing down to possible methods, and finally a discussion of priorities – quick return to work, contact sports, screen work, night driving.

Four Methods – How They Differ

SmartSight and SmartSight Nova – lenticule extraction, no flap

A femtosecond laser cuts a thin disk of tissue (lenticule) inside the cornea, which the surgeon removes through an incision a few millimeters long. No corneal flap is created, thus eliminating the risk of its displacement, and a larger part of the corneal nerves remain intact – this is significant for those prone to dry eye.

SmartSight Nova is a variant with CenTrax® centration on the visual axis: the system tracks eye alignment and corrects rotation (cyclotorsion) in real-time. This is most important for astigmatism, where a few degrees of rotation can change the outcome.

FemtoLASIK – corneal flap, fastest stabilization

A femtosecond laser forms a corneal flap, which is then lifted; a second laser reshapes the tissue underneath, after which the flap is repositioned. Vision usually stabilizes within 24–48 hours – this is still the fastest return to sharp vision.

Trans-PRK – surface treatment, no deep penetration

The laser works on the corneal surface, without an incision and without a flap. Healing takes longer (usually 5–7 days for comfortable vision), but the method uses the least tissue and remains an option where the cornea is too thin for other procedures. It is also the most resistant to mechanical injury – with no flap, there is nothing to displace.

Which Method Is Likely For You

  • Contact sports, physical work, uniformed services: flapless methods – SmartSight, SmartSight Nova, or Trans-PRK. No flap means no risk of its displacement in case of injury.
  • Tendency for dry eye: lenticular methods preserve most of the corneal nerves that control tear secretion.
  • Higher astigmatism: SmartSight Nova – real-time cyclotorsion correction has the greatest impact on the outcome here.
  • Priority is the fastest return to sharp vision: FemtoLASIK.
  • Thin cornea: Trans-PRK as the most tissue-sparing method; if even this is not an option, we consider phakic ICL lenses, which do not remove corneal tissue.

What No Method Can Do

Laser vision correction corrects refractive error, but it does not stop the aging of the eye. After 45 years of age, presbyopia develops, regardless of previous surgery. It also does not protect against cataracts or glaucoma – these are separate processes requiring separate treatment.

Honestly about the risks: each method can cause temporary dry eye, fluctuations in vision acuity in the first weeks, and night glare. Some patients may have a residual refractive error requiring enhancement. The risk largely depends on the quality of the qualification – which is why we do not shorten the diagnostic process.

Limitations:

  • no CenTrax system – for higher astigmatism, we usually recommend the Nova variant
  • hyperopia requires a different method

FemtoLASIK

Advantages:

  • fastest return to sharp vision – often the next day
  • the longest-studied method with a femtosecond laser, very predictable
  • corrects myopia, hyperopia, and astigmatism

  • corneal flap – caution with contact sports and in uniformed services

Limitations:

  • epithelium regrows in 3–4 days: tearing, photophobia, sick leave for 5–7 days

Three Example Scenarios

The following examples illustrate the thought process, not descriptions of specific patients. Measurements always determine the choice.

Thin cornea, -4 D refractive error, office work

Tomography shows the cornea is too thin to safely perform FemtoLASIK. Trans-PRK is the most tissue-sparing; for an even thinner cornea, we would consider ICL. The price for comfort: several days of healing and sick leave.

Uniformed services, -3 D refractive error with 1.5 D astigmatism

The risk of eye injury excludes methods with a flap. Astigmatism favors SmartSight Nova – real-time eye rotation compensation precisely targets the axis of the refractive error. Return to service after control and healing of the incision.

-2 D refractive error, need for quick return to work

Cornea is thick and regular, ocular surface healthy, no contact sports. FemtoLASIK provides the fastest stabilization – control the next day, computer work after 48 hours.

Which method is best for dry eye?

Lenticular methods – SmartSight and SmartSight Nova – because they preserve a larger part of the corneal nerves responsible for the tear reflex. The final decision is made after evaluating the tear film during the qualification examination.

What to do if the cornea is too thin for laser surgery?

We consider Trans-PRK as the most tissue-sparing method, and if even that is not an option – phakic ICL lenses, which correct the refractive error without removing corneal tissue and remain fully reversible.

All four methods have a serious complication rate below 1% if the qualification is thorough. Flapless methods (SmartSight, SmartSight Nova, Trans-PRK) eliminate the risk of flap displacement in case of injury. Trans-PRK is the most tissue-sparing. Safety depends more on the quality of the examination than on the method’s name.

Which method causes the least pain and heals the fastest?

The procedure itself is painless with every method – the eye is anesthetized with drops. The difference concerns the first few days: after FemtoLASIK and SmartSight, comfortable vision returns in 24–48 hours; after Trans-PRK, the epithelium regrows in 3–4 days, and during this time, tearing, photophobia, and a gritty sensation under the eyelids are possible.

Is it worth choosing the cheapest method?

Trans-PRK for 10,000 PLN is the cheapest, but not because it is inferior – it is simply technologically simpler and takes longer to heal. For a thin cornea, it can be the only choice, but for someone who needs to return to work in two days, it’s a poor one. Price should not be the number one criterion; the measurement results should be.

Can I choose the method myself?

You can have a preference, and we discuss it during qualification. However, the final choice results from measurements: corneal thickness and topography, degree of refractive error, and ocular surface condition. If your preference is safe, we perform it.
