The Administrator processes personal data in order to schedule a treatment – the basis for processing is to take action at the request of the Service user, prior to entering into a contract, and possibly to establish and assert or defend against claims – the basis for processing is the Administrator’s legitimate interest. Optional personal data is processed on the basis of consent. Personal data will be processed for a period of 6 months after sending the message, after which time it may be processed until the statute of limitations for possible claims. Using the “make an appointment” tab requires you to enter your name and phone number. Optionally, you can also provide an email address. Provision of the required personal data is voluntary, nevertheless, failure to provide it prevents contact with the Administrator. Providing an email address may improve contact with the Administrator.
